Anthem Blue Cross Select HMO and Traditional HMO – Basic HMO Plans
Anthem Blue Cross offers online tools, benefits, discounts, and programs to enhance your well-being. For questions about these benefits, contact Member Services at the number listed on the back of your membership card.
- Sydney Health App: Manage your health plan, find providers, check costs, review coverage details, and access your digital ID card in one app. Additionally, you can access 24/7 video visits with doctors, therapists, or psychiatrists for the same copay as an office visit, with Spanish-speaking doctors available.
- Health Record: Keep all your health information, including conditions, immunizations, tests, and medications, safe and organized in one place to easily share with your doctors.
- Diabetes Prevention Program (DPP): Through December 31, 2026, eligible Anthem members with pre-diabetes may be able to join a 12-month program with in-person or online sessions to help prevent diabetes available through Lark. Beginning January 1, 2027, Anthem’s DPP will be powered by Teladoc. This program offers eligible members a wireless scale that uploads your information to the app automatically, 24/7 digital coaching, and personalized support to help lower your risk of type 2 diabetes.

- Building Healthy Families Program: This program provides personalized support for family planning and pregnancy through the toddler years, including access to maternity nurses, virtual lactation support, and 24/7 digital tools via the Sydney Health app or Anthem website. Resources are also available for adoption, surrogacy, and single parenthood.
- LiveHealth Online: LiveHealth Online offers virtual visits with board-certified doctors and licensed therapists, providing care for conditions like colds, sinus infections, or mental health support.

- The House Call Program: Receive an annual in-person or virtual health evaluation from a licensed clinician, including medication review and help with your follow-up appointments.
- Medicare Community Resource Support: This benefit connects you to local community resources and support services such as transportation, copay, food and utility assistance, and durable medical equipment, social activities, and housing resources.
- TruHearing: You can access hearing exams, discounted digital hearing aids, and related services at more than 4,500 locations nationwide through TruHearing. For more information, call (855) 312-2545.
- Personal Emergency Response System: You can receive an in-home emergency alert device to help maintain independence and safety in your home.
- Healthy Meals: Eligible members recovering from inpatient stays or with qualifying conditions can have healthy, fully prepared meals delivered to their homes.
- Routine Transportation: Access transportation for up to 12 non-emergency one-way trips (limited to 60 miles per one way trip) per year within the local service area for plan-approved health related visits, including medical appointments, SilverSneakers locations, and pharmacy trips.
- Cancer Care Navigator: Specialized support is available to help members understand their cancer diagnosis, treatment options, benefits, and connect to community resources.
Blue Shield Access+ and Blue Shield Trio Basic HMO Plans, and Blue Shield of California EPO Plan
Blue Shield of California offers extensive health information, well-being tools, and discount programs.
- Wellvolution: Wellvolution offers personalized, digital health coaching programs to help you improve your health and achieve your goals. Programs are available for diabetes care, weight management, physical therapy and fitness, tobacco cessation, and emotional well-being.
- Preventive Care: Use Blue Shield’s interactive preventive care tool to create a customized list of topics to discuss with your doctor and to stay current on recommended screenings, tests, and immunizations based on your age, gender, and health history.
- Diabetes Prevention Program: This 12-month program can help you lose weight and reduce your risk of type 2 diabetes. If you’re eligible for the Diabetes Prevention Program, you can receive a Fitbit, digital scale, and health coaching at no additional cost.
- Maven Maternity Support: Maven programs are designed to support you and your family through family planning, fertility, maternity, parenting, pediatrics, menopause, and pregnancy loss. You can access virtual appointments, a care advocate, and educational resources.
- Discount Programs: Through Live Healthy, you can get discounts on fitness programs, vision services and LASIK, massage therapy, acupuncture and chiropractic services, and hearing aids.
- Fitness Your Way® by Tivity Health: If you’re 18 years or older, you can access a nationwide gym discount program with multiple fitness packages options through Fitness Your Way®. The program includes thousands of fitness locations nationwide.

- Real Age Test (Health Assessment): The RealAge Test gives you a personalized report of behavioral and medical health risks and a customized action plan.
- Craving to Quit (Tobacco Cessation): The Craving to Quit program provides access to one-on-one phone coaching and online resources to help you quit smoking, dipping or vaping.
Maternity and Family Planning: Health Net offers support through pregnancy, labor, delivery and postpartum. This includes members who have experienced a stillbirth, miscarriage or abortion.
If you are interested in doula services, you can choose either the Individual Doula Program or Mahmee Virtual Doula with Wrap-Around Services.
Additionally, the Start Smart for Your Baby program is available to eligible members to provide education and support to make healthy choices during pregnancy.
Unwinding by Sharecare: Unwinding is a mindfulness app that helps you manage stress, build resilience, improve sleep, and support your mental health through guided exercises and tools.
Eat Right Now: Eat Right Now is a virtual program with tracks for diabetes prevention, weight loss, and mindful eating. It includes guided lessons, coaching, mindfulness exercises, live weekly calls with a behavioral change expert, and other support services.
Health Coaching: The Health Coaching Program offers one-on-one phone health coaching and resources to support goals like weight loss, healthy eating, stress management, exercise, and tobacco cessation.
Rewards: Eligible members can earn annual gift card rewards of up to $100 through the Health Net Sharecare Wellness Rewards Program by completing specific health activities.
Kaiser Permanente – Basic HMO Plan and Medicare Advantage Plans
Kaiser Permanente Health and Wellness resources offer self-care apps, health classes and programs, fitness deals, and wellness coaching.

- Health and Wellness Resource Hub: Use this one-stop hub to find wellness programs and services, including health guides, online and facility-based classes, assessments, tips, calculators, podcasts, and videos. Start by taking the Total Health Assessment.
- Health Calculators: Use Kaiser Permanente’s health calculators to check things like healthy weight, calcium intake, and how to calculate your due date if you’re pregnant.
- Diabetes Prevention Program: You have access to online resources and classes at medical centers to help prevent and manage diabetes. Kaiser also partners with Omada Health to offer a no-cost Diabetes Prevention Program for eligible members age 18 and older, which includes a health coach, a wireless scale, peer support, and other tools. The program has the following eligibility criteria:
- A body mass index (BMI) of 25 or greater (for self-identified Asians, the BMI threshold is 23 or greater)
- A hemoglobin A1c test with a value between 57% and 64%, or a fasting plasma glucose of 110 to 125 mg/dL, or a two-hour post-glucose challenge of 140 to 199 mg/dL (oral glucose tolerance test)
- Self-reported previous diagnosis of gestational diabetes
- No previous diagnosis of type 1 or type 2 diabetes
- Understanding Mental Health: Access mental health and addiction care by visiting the Northern California or Southern California Get Care page to explore your care options.
- Mental Health and Wellness Digital Self-Care Tools: Use the Calm and Headspace apps to support your mental health and emotional well-being.

- One Pass Select Affinity: One Pass Select Affinity is a subscription-based fitness membership that helps you reach your health goals. It includes access to gyms and studios, digital and on-demand workouts, personalized coaching tools, and grocery and household delivery.
- Affinity Musculoskeletal Program: Get help with your pain through Optum’s Affinity Musculoskeletal Program, which offers access to complementary and alternative providers, preferred rates, and support for health decisions. The network has thousands of providers who practice acupuncture, massage therapy, and chiropractic care.

- Second Medical Opinions: With Included Health’s second-opinion service, a top specialist reviews your diagnosis or current treatment plan so you can better understand your options.

- Care and Case Management: Included Health’s integrated Care and Case Management Program provides whole-person care and dedicated support for complex health conditions like diabetes, back pain, and pregnancy. Sharp Health Plan’s prevention and wellness program offers prevention and wellness resources, plus a comprehensive wellness program designed to support your health goals.
- Best Health: Best Health is a wellness program available to all Sharp Health Plan members that includes online wellness tools and interactive learning modules, one-on-one health coaching, and fitness and wellness discounts.
- Primary subscribers can earn a $150 Best Health eReward by completing the Wellness Assessment and an additional qualifying wellness activity to earn your reward.
- Diabetes Prevention Program: The Sharp Rees-Stealy Diabetes Prevention Program is a 12-month program for members with pre-diabetes. It offers clinical and lifestyle coaching, educational sessions to optimize behavioral change, and online tools to develop goals. Eligible participants must:
- Be at least 18 years old
- Have a body mass index (BMI) of at least 24 (22 if Asian)
- Blood tests within the last year that place them in the pre-diabetes range.

- Doula Care Program: Mahmee provides personalized support throughout pregnancy, delivery and the postpartum period. Mahmee's team includes doulas, registered nurses, mental health coaches and nutritionists, offering a range of services through virtual visits, in-person care, 24/7 support and more. - Health and Wellness Site: Sutter’s Health and Wellness site supports both your physical and mental health, with tools and resources designed to help you reach your health and wellness goals. It includes a personal health assessment, 13 action plan modules, and a Health Library with learning centers, videos, a symptom checker and more.
- Virtual Health Program: Sutter’s Virtual Health Program supports weight management and helps prevent or manage chronic conditions such as diabetes, high blood pressure and high cholesterol, with video visits, secure in-app messaging, and tracking for food intake, exercise and sleep habits.
- Sutter Care Management Program: The Sutter Care Management Program offers confidential health coaching to support healthy weight, tobacco cessation and stress management. You’ll work one-on-one with a coach by phone to identify barriers, explore how your behaviors align with your personal values, and set realistic goals.

- UnitedHealthcare Rewards — Wellness Incentive Program: With UHC Rewards, you can earn up to $300 each for you and your covered spouse/domestic partner by completing activities such as tracking steps or active minutes, sleep tracking, going paperless, getting a biometric screening, taking a health survey, or connecting a tracker.
- Preventive Care Resources: Make preventive care a priority by utilizing UHC’s preventative care checklist, resources, and working with your doctor to create a preventive care plan for the year.

- OnePass Select™: OnePass Select™ is a flexible subscription-based fitness and well-being program offering access to thousands of nationwide fitness networks and digital access through the UHC app or myUHC.com.
- Virtual Visits: Talk to a doctor online, without an appointment, about medical questions, diagnoses, prescriptions, and support for addiction, depression, trauma, loss, stress, or anxiety. Virtual Visits are available through designated providers like AmWell, Doctor on Demand, and Teladoc Health.
UnitedHealthcare Group Medicare Advantage (Medicare PPO) Plan

- Let’s Move: Let’s Move offers content and programs focused on nutrition, fitness, mental health, social well-being, caregiver support, financial wellness, and tobacco cessation.
- Medicare Fitness Program: UHC provides a no-cost fitness program that includes a gym membership at a participating fitness center, on-demand workout videos and live streaming classes, and access to local health and wellness classes and events. This program will be provided through the UHC Medicare Fitness Club Membership program until December 31, 2026. On January 2027, the same benefits will be offered via Renew Active.®
- Optum® HouseCalls: Optum® HouseCalls is a free in-home health and wellness program that provides up to an hour with a licensed practitioner for a physical exam, health screenings, medication review, health advice and resources, and referrals if needed.
- Virtual Visits: Virtual Visits let you video chat with a provider any time day or night. Note: Virtual visits are not for use in emergencies.
- Calm Health App: The Calm Health app offers self-paced programs and tools to support your mental health and well-being.
- Real Appeal® Weight Management and Real Appeal® Diabetes Prevention Programs: The Real Appeal® online programs are designed to help eligible members reduce the risk of developing serious health conditions and achieve their health goals through behavior change strategies and support. The programs include online coaching and resources, a success kit, tracking tools, and mobile app.
- UnitedHealthcare Healthy at Home: Healthy at Home provides up to 30 days of support after inpatient or skilled nursing facility discharge, including 28 home-delivered meals, 12 one-way rides to medically related appointments and the pharmacy, and 6 hours of non-medical personal care assistance. Meal and transportation benefits require a UnitedHealthcare Engagement Specialist referral, which must be renewed after each discharge.
- CareLinx In-Home Personal Care: You are eligible for 16 hours of non-medical personal care through CareLinx, which may include grocery shopping, meal preparation, transportation, personal care, medication reminders, and more.
Western Health Advantage (Basic HMO)
Western Health Advantage (WHA) provides access to preventive health information and resources.
- MyWHA Wellness Online Portal: MyWHA Wellness helps you set wellness goals, track progress, and access personalized health resources like recipes, videos, and articles. You can also take a personal health assessment to receive a wellness score, along with a report about health risks.
- Preventive Care Resources: WHA’s preventive guidelines include screenings, tests, and other services available at no additional cost, along with CDC-recommended immunization schedules from birth through adulthood. These guidelines help you make informed decisions about your health. Coverage for WHA services depend on eligibility at the time of service.
- Vitality Diabetes Prevention Program: Western Health Advantage’s diabetes prevention program through Vitality is a one-year lifestyle program for CalPERS members with prediabetes that is designed to slow or prevent type 2 diabetes. It includes online group coaching, digital progress tracking, rewards for completing milestones, and physical, emotional, and behavioral health guidance.
- Virta Diabetes Reversal (type 2): Through Virta Health’s Type 2 Diabetes Reversal Program, you can access a telehealth program that helps lower blood sugar and A1C, reduce or eliminate medications, and lose weight. It includes a medical coaching team and all supplies needed for the program.
- Condition Management Programs: You have access to 12-week programs to help you manage chronic or high-risk health conditions such as asthma, coronary artery disease, diabetes, congestive heart failure, and chronic obstructive pulmonary disease. This program will be offered through Optum Health until December 31, 2026. Beginning January 1, 2027, the program will be managed by Vitality.
- Hypertension Management: The Livongo® program helps eligible members manage high blood pressure and reduce risk with real-time readings, advice, and personal lifestyle coaching.
- Pregnancy Support: You and your partner can access in-person and virtual doula care services via the Pregnancy and Postpartum Program. Additionally, the Maven Maternity program provides virtual support from doctors, specialists, and coaches.
- Pain Management: Ease pain and support recovery with acupuncture and chiropractic care. These services can help with back and joint pain, headaches, sprains and strains, muscle spasms, and more. Access 20 combined visits annually through WHA’s network partner, Landmark Health - no referral needed.
- Instructor-led Classes and Support Groups: WHA network’s medical groups sponsor health education programs, support groups, and classes for diabetes, fitness, heart and vascular, lung health, nutrition, orthopedics, parenting, pregnancy and childbirth, and smoking cessation. Unless noted, most health programs or classes have no additional cost.
- Gym and Fitness Center Discounts: Active&Fit Direct™ and ChooseHealthy® offer discounts on fitness centers, classes, products, and services. Area partners include California Family Fitness, and Active Wellness Centers in Petaluma, Napa, and Vacaville. Visit MyWHA for more details.
- CommunityFIT: CommunityFit is a community-based fitness program that offers bite-sized virtual classes designed to help you move more, recharge, or learn something new.
- Healthy and Delicious Recipes: A nutritionally sound lifestyle can help reduce the risk of infection and disease, improve emotional well-being, support healthy weight management, and promote longevity.
