Dependent Eligibility Verification Project

We know health care coverage is important to you and your entire family. As part of our commitment to control health care costs, CalPERS is conducting a project to ensure dependents enrolled in the health benefits program meet the plan eligibility guidelines so that only eligible dependents are covered. Therefore, it is important to validate that your health care dependents meet CalPERS eligibility criteria and that you make necessary enrollment changes in a timely manner, if applicable.

Amnesty

The initial phase of the DEV project included an amnesty period that ran through June 30, 2013. During amnesty, health plan subscribers could voluntarily disenroll ineligible dependents on a prospective (future) basis.

The deadline to complete amnesty disenrollments was June 30, 2013.

Verification

Verification is the second phase of the DEV project. During this time, subscribers will provide documentation to our contracted vendor for the project, HMS Employer Solutions (HMS), to verify that the dependents enrolled in their health plans are eligible to receive the benefit.

Verification will occur in cycles starting in July 2013. During your verification cycle, you will receive a letter from HMS with details and instructions on how and where to submit this documentation. HMS has performed more than 800 verification programs like the one we are undertaking, and verified more than 3.5 million dependents for some of the largest employers in the country. HMS will securely store and protect all documents received during the DEV project in compliance with applicable federal and State regulations.

Supporting Documentation

The following list will help you identify the required documents for each eligible dependent. All required documents MUST include date and/or year, subscriber name and dependent's name.

Eligible Dependents Required Documentation
Current spouse A copy of your marriage certificate AND one of the following: a copy of the front page of your 2012 federal or state tax return confirming this dependent is your spouse OR a document dated within the last 60 days showing current relationship status, such as a recurring household bill or statement of account. The document must list your name, your spouse's name, the date and your mailing address.
Note: Proof of common residency is for the purpose of establishing current relationship only. Common residency is not a CalPERS requirement for health benefits eligibility.
Current registered domestic partner A copy of your Declaration of Domestic Partnership AND one of the following: a copy of the front page of your 2012 state tax return OR a document dated within the last 60 days showing current relationship status, such as a recurring household bill or statement of account. The document must list your name, your partner's name, the date and your mailing address.
Note: Proof of common residency is for the purpose of establishing current relationship only. Common residency is not a CalPERS requirement for health benefits eligibility.
Natural, adopted, step or domestic partner's children up to age 26 A copy of the child's birth certificate (or hospital birth record) or adoption certificate naming you or your spouse as the child's parent OR a copy of the court order naming you or your spouse as the child's legal guardian.
Note: For a stepchild, you must also provide documentation of your current relationship to your spouse or domestic partner as requested above.
Children up to age 26, if the employer has certified that the employee has assumed a parent-child relationship and is the primary care parent A copy of the front page of your 2012 federal or state tax return confirming this dependent OR a copy of the court order naming you or your spouse as the child's legal guardian OR day care receipts or school records which indicate the child resides at your current mailing address, if applicable.
Certified disabled dependent children of any age Not applicable. CalPERS will not be asking for documentation for these dependents during the DEV project.
